Obesity is a chronic disease that poses a significant threat to health throughout one's life. Obesity is closely associated with insufficient physical activity (PA). It is widely acknowledged that exercise is an effective strategy for preventing and managing obesity. However, China still lacks guidelines for exercise in obesity and standardized exercise prescriptions based on national evidence. Cohort studies, as an epidemiological research method for exploring the causes of diseases and evaluating the effects of prevention, are a fundamental support platform for the formulation of health policies and the development of clinical practice. In this paper, a comprehensive literature review was first conducted to identify high-quality cohort studies that examine the prospective relationship between exercise/PA and the development of obesity. The findings indicated that the United States, the United Kingdom, and Australia have the most abundant cohort resources in this field. Subsequently, the international classic cohorts were focused on, and the construction experience was summarized from the aspects of subject selection, recruitment, data collection, and follow-up monitoring design. In light of the current state of research in China, it is proposed to implement a comprehensive population-based exercise cohort study that covers diverse regions across the country, spans the entire lifecycle, utilizes a multidimensional index system, and incorporates long-term follow-up.
